While Sanofi’s diet drug Rimonabant (Acomplia / Zimulti) remains stalled at the FDA, a potential competitor currently in phase III trials, Merck’s MK-0364, now has an official generic name: Taranabant.

Taranabant is Merck’s cannabinoid-1 (CB-1) receptor inverse agonist, and from reports we periodically receive from participants in the phase III trial of this diet drug, taranabant is as effective for weight loss as rimonabant with fewer side effects.

An experimental weight loss medication in the same class as the drug Acomplia helped patients lose weight in a 12-week, phase II study, but side effects were common at higher doses.

At the end of 12 weeks, the placebo-treated participants had lost the least weight and those treated with the highest dose of taranabant had lost the most.

Patients treated with 0.5 milligrams of the drug lost an average of 3.5 pounds, compared with 5 pounds among people treated with 2 milligrams of the drug and almost 9 pounds among those treated with 6 milligrams.

A total of 27% of patients who took 0.5 milligrams of taranabant lost 5% or more of their body weight, compared with 61% in the 6-milligram group.

But patients taking the higher dosages of the drug also had higher rates of side effects, including anxiety, nausea, and vomiting.

The new diet pill, if approved, won’t be for everyone. “This is not going to be a class of drugs that people take because they want to look good on vacation” and lose 10 pounds or so.

The European Director of the clinical trials, Professor Luc Van Gaal of the University Hospital in Antwerp, Belgium, presented results of this study at the 4th International Symposium on Obesity and Hypertension at the Max Delbr?nter for Molecular Medicine (MDC) Berlin-Buch.

Experiments with obese lab rats have shown that, the more they eat, the more endocannabinoids bind to the CB1-receptor, which is hyperactive in these animals. The result: The binding of endocannabinoids to CB1 increases appetite, Professor George Kunos demonstrated. He is from the National Institute on Alcohol Abuse and Alcoholism (NIAAA), which is part of the National Institutes of Health (NIH) in Bethesda, Maryland, USA. Furthermore, Professor Kunos, who was unable to attend the Berlin meeting, showed in experiments with mice that blocking this receptor with the substance rimonabant reduces appetite. The animals become thin and stay that way despite being offered an overabundance of food. And rimonabant not only reduces the animals’ weight but also their blood pressure.

For over two years, rimonabant has been tested in clinical trials. Director of the RIO (Rimonabant in Obesity) clinical trial in Europe is the diabetologist and metabolism expert Professor Van Gaal. Included in the trial are 1,507 patients from 60 clinics in Belgium, Germany, Finland, Sweden, the Netherlands, and the US with a Body Mass Index (BMI)* of over 30 kg/m2 and of over 27 kg/m2 who additionally have high blood pressure and elevated blood lipid levels. The RIO trial is one of a total of four Phase III trials (testing effects and side effects in a larger number of patients) with 6,600 patients. The patients were put on a diet and also had to complete an exercise program. They were divided into three groups: the first group received 20 mg of rimonabant daily, the second group received 5 mg, a day and the third group received a placebo.

Reduces Bodyweight and Improves Metabolic Parameters As Professor Van Gaal reported in Berlin, one year later, patients who had received daily doses of 20 mg of rimonabant had lost 6.6 kg on average; patients who took the lesser dose of 5 mg lost an average of 3.4 kg; patients who received the placebo lost only an average of 1.8 kg. The group of patients with a daily dosage of 20 mg of rimonabant not only lost the most weight of all of the trial participants, but they lost it in the critical places of abdomen and waist. It is remarkable, according to Prof. Van Gaal, that in these patients, the risk factors for metabolic syndrome and cardiovascular diseases were reduced more than could be expected from mere loss of weight. He estimates that 50 percent of this effect is due to rimonabant. The reason: while patients who received the 5 mg dose of rimonabant clearly lost weight, their blood lipid levels were not as improved as in patients with the higher dose.

We all know pill popping isn’t the answer when it comes to losing weight and keeping if off. But clinical trials with a new anti-obesity drug look promising. The drug, called rimonabant, was developed after scientists noticed that cannabis smokers often had increased appetites and frequently experienced the ‘munchies’.

They worked on the idea that if substances from cannabis called cannabinoids stimulated appetite, blocking the cannabinoid receptors in the brain might reduce appetite.

Furthermore, blocking these receptors might also help people to give up smoking as these receptors not only appear to be involved in controlling food intake, but also have a role to play in tobacco dependence.

Earlier trials with the drug, which works by blocking cannabinoid receptors in the brain, have already shown promising results. But now a new study published in medical journal The Lancet, confirms the drug can help with weight loss and reduce the risk factors associated with heart disease.

In the study, more than 1,500 obese people were given a calorie-controlled diet, together with either 5mg or 20mg of rimonabant each day or a dummy drug. After one year, two thirds of the participants taking the largest dose had lost at least 5 percent of their weight. Better still, of these, 39 percent lost more than 10 percent. Meanwhile, patients on the 20mg dose lost on average, 4cm from their waist and risk factors for heart disease including raised cholesterol, insulin resistance and the presence of metabolic syndrome improved.

Professor Luc Van Gaal who led the study says, “In this study, treatment with rimonabant over one year led to sustained, clinically meaningful weight loss, reduction in waist circumference and associated improvements in several cardiovascular risk factors.”

For the next three or four years Sanofi-Aventis sais that they will not try  to sell diet medication acomplia rimonabant in the United States. The company declared that on Sept. 17th. But they also showed that they still have  high expectations for acomplia rimonabant, specially as a treatment for type 2 diabetes.

Sanofi’s head of research Marc Cluzel said:”I believe  in Acomplia rimonabant because it is a unique product,” this happened in front of all the reporters and analysts at a presentation on the company’s research and development program.

Meanwhile Acomplia Rimonabant was launched  Europe’s market in 2006 and it is now  available also in of other states,the producer of the drug, Sanofi, retired its application,that had the purpose to acced with acomplia in the United States ,after  FDA  unanimously recommended the rejection of  the drug because of the side-effects including depression and suicidality.

Sanof declared that they planned to use approximately 11,000 of the 17,000 people for this trial, and they have clues that the result  this study will be available to share with the FDA in 2010.

Cluzel declared that  Sanofi expects to refile its application with the FDA after longer-term data on the use of Acomplia rimonabant has been gathered from the large CRESCENDO trial, which got underway in December 2005.

But despite the fact Sanofi got some bad news related with the near-term future of Acomplia rimonabant as a diet drug, the company said that their new plans are now to aggressively  press ahead with some new studies in order to aimed gain the  approval for using acomplia rimonabant as a main-line treatment of type 2 diabetes.
